Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 11/30/20 and concluded on 12/2/20. The provider was contacted by telephone to initiate the inspection. There were 2 children present with the provider for a total of 3 points. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 provider/household member records submitted by the provider to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law in the area of administration and violations were documented on the violation notice issued to the provider.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-111-90-C
Description: Based on record review and interview, the provider did not ensure that documentation of additional immunizations are obtained once every six months for children under the age of two years.

Evidence:
1. The most recent immunization record on file for child 1, age 10 months, is dated 3/17/20. Child 1 was due for an updated immunization record in September 2020.
2. The provider confirmed that updated immunization records for child 1 were not obtained within the required time frame.

Plan of Correction: The provider responded with the following: The provider will contact the parent to obtain updated immunization records for child 1.
