Comments:
An unannounced renewal inspection was conducted on 11/01/2022. There were 5 children, ages 3months ? 3 years, totaling 15 points and 1 caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 2 children?s records, 1 caregiver?s records and 1 household member?s records were reviewed. The children were observed during center time. The inspector arrived for the inspection at 10:20am and departed at 11:43am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on the review of one caregiver?s record, the provider failed to ensure that every adult living in a family day home shall undergo all background checks in accordance with the Code of Virginia prior to the issuance of a license and every five years thereafter.
Evidence: The most recent sworn statement in the record for Caregiver 1 was signed and dated 12/17/2015. There was not a signed updated sworn statement completed in 2020.
Plan of Correction:
Provider will complete an updated sworn statement.
