Comments:
An unannounced monitoring inspection was conducted on 11/27/2023. There were 3 children, age 5 months to 4 ? years, totaling 9 points, 1 caregiver and 1 household member present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 2 children?s records , 1 caregiver?s records and 1 household member?s records were reviewed. The children were observed during self-directed play. The inspector arrived for the inspection at 9:50am and departed at 10:48am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on the review of two children?s records, the provider failed to ensure that each report shall be signed by a physician, the physician?s designee, or an official of the local health department.
Evidence: 1. Child #2 start date was 10/05/2023 and on the day of the inspection did not have a physical record signed by a physician. 2. Child #2 start date was 10/05/2023 and on the day of the inspection did not have an immunization record signed by a physician.
Plan of Correction:
Provider will obtain a signed physical and immunization record from parent.
