There were 15 children present with a teacher and a substitute. The children were playing outdoors. Two child files and two staff files, the physical plant, staffing supervision and emergency drills were reviewed. Thank you for your cooperation. Please contact me if I may be of service. Violations: Standard #: 8VAC20-770-60-B Description: Based on a review of staff files, the facility failed to ensure that the correct background check form is used. Evidence: 1. The sworn statement on file for staff #1 does not ask for previous states of residence for the last five years. Plan of Correction: The program manager will ensure that all background checks are completed on the correct forms.
