4/14/23 Arrived 9:50am Departed 10:50am An unannounced renewal inspection was conducted today. There were 15 preschool children (ages 3-5yrs) present with one classroom staff member and a substitute. The children were playing in centers, eating lunch and playing outdoors. Two child records were reviewed. Staff records will be reviewed at the main office. Thank you for your cooperation. Based on a review of staff files, the facility failed to ensure that all employees complete a sworn statement prior to employment. Evidence: 1. There is no fingerprint back ground check or sworn statement on file for staff #3 who is working in the classroom today.
Plan of Correction:
Fieldprint appointment is scheduled for 4-28-23. Sworn statement has been signed. 4-26-23
