Comments:
A monitoring inspection was initiated on 4/11/24 and concluded on 4/11/24. There were 17 children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records were reviewed. The inspection started at 12:10 pm and concluded at 1:35 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record reviews the center failed to ensure that each center shall maintain a separate record for each child which will contain documentation of viewing proof of the child's identity and age. Evidence: Child # 2 began care 8/14/23. There is no documentation of viewing proof of birth for this child.
Plan of Correction:
The center will ensure that all the required documentation is in the file within the set timeframes.
