Comments:
Today an unannounced Monitoring Inspection took place between the hours of 10:20am and 11:40am. There were 2 classrooms observed with a total of 28 children with 3 staff within the supervision guidelines. The children were observed eating their morning snack, playing outside and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log and medications and medication authorizations were observed during this inspection. Based on record review and interview, the child day center has not obtained a central registry background check for each employee every five years. Evidence: Staff #4's most recent central registry background check is dated 6/8/2015. The center Director confirmed that the repeat background check had not been completed.
Plan of Correction:
I will print the form and have the staff complete the background check request and we'll have it in the mail tomorrow.

Description:
Based on record review and interview, the child day center has not obtained an out-of-state central registry background check or out-of-state sex offender background check for each employee that has resided outside of the state of Virginia within the preceding five years. Evidence: Staff #1's sworn disclosure statement documents that the employee has resided outside of Virginia with the last five years. The program Director confirmed that out-of-state background checks have not been completed for Staff #1.
Plan of Correction:
The out-of-state sex offender background check was completed online today. The employee will complete the central registry background check as soon as possible.
