A monitoring inspection was conducted on 8/16/19 from 10:50 am until 2:00 pm at Lindale Minnonite Church to review religious exempt requirements. At the time of the inspection 56 children were present with 11 staff. The sample size consisted of six children's records and nine staff's records. Children and staff were observed during free indoor and outdoor play, during a science experiment, clean-up, lunch preparations, hand washing, bathroom break, attending to a sick child, pick-up, transitions and behavior management. Based on observation and interview, the center failed to post the fact that the program is exempt from licensure in a visible location.
Evidence:
1. During the tour of the facility the center's exempt status was not observed posted in the facility. 2. The director stated they have not posted the center's exempt status.

The director will post a statement stating the center is exempt from licensure.

Description:
Based on interview, the center failed to establish and implement a procedure to ensure that all staff are able to recognize the signs of child abuse and neglect.
Evidence:
1. The records for nine staff were reviewed. No documentation was found stating staff had completed training in recognizing child abuse and neglect. 2. The director stated staff are not trained upon hire in recognizing child abuse and neglect.
Plan of Correction:
All current staff that have not completed training in recognizing child abuse and neglect will be required to complete the training. All new staff will be required to complete training in recognizing child abuse and neglect upon hire.
