An unannounced monitoring inspection was conducted on April 30, 2024. This on-site visit was conducted from 9:50 a.m. through 10:35 a.m. Seventeen children, ages 3-5, were in the care of three staff. Five children and five staff records were reviewed.
Areas of non-compliance may be found on the violation notice. Please complete your plan of correction within 5 business days and return to the licensing inspector.

Based on record review the facility failed to obtain out of state central registry check within 30 days of hire.
Evidence:
