Comments:
The licensing inspector conducted an unannounced monitoring inspection from 10:10 am - 12:15 pm. There were ten children in care for a total of twenty eight points with three caregivers. The children were observed playing with toys and watching cartoons. Three caregiver's records and six children's records were reviewed. Areas of non-compliance are identified on the violation notice. Based on record reviews, the provider failed to obtain a copy of a child's physical. Evidence: 1. The inspector reviewed six children's records. One of six records reviewed (child record #2) did not contain documentation of a physical and child's date of enrollment was 09/10/2019.

The provider will remind the child's parents that a copy of the child's physical is needed for the child's record.
