A complaint inspection was initiated on October 27, 2023 in response to a complaint that was received on October 25, 2023 in reference to ratios and supervision. Documentation was gathered and interviews were conducted. The preponderance of evidence gathered during the investigation supports violation of the Code of Virginia in regard to the complaint.
Based on interview, and documentation review, the center failed to follow staff to child ratio of one staff member to 10 children from ages 36 months to five years. Evidence: Review of daily attendance for the months of September and October 2023 revealed that there were multiple days where there were up to 15 children, age 4 years, that attended. Interview with staff #1 and administration confirmed that classroom #1 frequently has 1 staff member with up to 15 children, age 4 years, at a time.
