Comments:
An unannounced monitoring inspection was conducted on June 7, 2023 with the provider. The inspection began at approximately 10:00am and concluded at 11:30pm. There were 9 children present, ranging in ages from 2 years to 8 years, totaling 16 points with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records, 1 staff record, and 1 household member?s record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Based on record review and interview, the provider failed to ensure that all employees shall undergo background checks every five years. Evidence: 1. The staff record for Staff #1 contained a Central Registry background check dated 8/9/2017. Staff #1 should have undergone a criminal background check on or before 8/9/2022. 2. The provider verified that a central registry background check had not been completed for staff #1 since 2017.
