Self report violations An unannounced self reported inspection was conducted regarding concerns of the center following their own transportation policies and procedures. Two centers were visited to interview management between 9:30am-12:00pm. Based on the evidence gathered, violations were cited. For violation details, please refer to the Violation Notice. Based on interview with center management, center failed to follow their own transportation policies and procedures. Evidence: On Friday, October 18, 2019, the bus driver left the elementary school without confirming all children scheduled to be on the bus, were actually on the bus.
Plan of Correction:
We will review again our transportation polices and procedures with all drivers.
