Technical Assistance:
Inspector spoke with Director about one of the two transportation buses expired Virginia State Inspection stickers. The bus is currently not in use.

Comments:
An unannounced monitoring inspection was conducted July 3, 2024 between the hours of 10:00am - 12:15pm. There were 77 children directly supervised by 12 staff. The physical plant, 5 staff records, 8 children?s records, evacuation drills, injury reports and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating lunch. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within five business days in order for it to be included on the public website.

Description: Based on observation, the center did not ensure areas and equipment of the center, outside, were maintained in a clean, safe, and operable condition.
Evidence:
1. On the Toddler playground, a plastic storage shed was observed having broken and detached doors exposing sharp pieces of plastic at the top.

Plan of Correction: Work order placed to secure doors.

Description: REPEAT VIOLATION and SYSTEMIC DEFICIENCY

Based on observation hazardous substances were not kept in a locked place using a safe locking method that prevented access by children.
Evidence:
1. In the School age classroom, fourteen (14) bottles of cleaning supplies were observed on lower shelves in an unlocked cabinet.
2. In the first bathroom in the School age classroom, one (1) bottle of cleaning spray was observed on the high shelf above the sink.
3. In the second bathroom in the School age classroom, one (1) bottle of cleaning spray was observed on the high shelf above the sink.
4. In the Pre-K classroom, one (1) bottle of cleaning spray was observed on top of a cabinet.
5. In the first bathroom in the Pre-K classroom, two (2) bottles of cleaning spray were observed on the high shelf above the sink.
6. In the second bathroom in the Pre-K classroom, three (3) bottles of cleaning spray were observed on the high shelf above the sink.
7. In the Discovery Preschool A classroom, one (1) bottle of cleaning spray was observed on top of the cubbies,

Based on observation, the diapering surface was not used only for diapering or cleaning children.
Evidence:
1. The diapering surface in the Discovery Preschool B classroom was used to store children's clothes and shoes during the time of the inspection.

Plan of Correction: Retrain staff on regulations on changing table cleanliness.

Description: Based on observation, over the counter skin products were kept beyond their expiration dates.
Evidence:
1. In the Discovery Preschool B classroom, five (5) expired diaper creams were observed in the bin under the diaper changing table.

Plan of Correction: Return diaper creams to parents.

Standard #: 8VAC20-780-520-C
Description: Based on observation, diaper cream was observed accessible to children.
Evidence:
1. In the Discovery Preschool B classroom five (5) ,diaper ointments were observed in an unlocked cabinet under the changing table accessible to children.
2. In the Infant B classroom, six (6) diaper ointments were observed in an unlocked cabinet under the changing table accessible to children.

Plan of Correction: Ensure staff locks cabinet after each diaper change.

Description: Based on observation, prepared infant formula was not dated.
Evidence:
1. In the Infant B classroom refrigerator, three (3) infant formula bottles for Child # 9 were not dated.

Plan of Correction: Follow up with parents regarding bottle label dates.
