Technical Assistance:
Technical assistance today included record keeping Make sure to update all phone numbers for the current 800 number for Poison Control. We also discussed the alignment of a piece of the yellow slide where it meets the platform.
Comments:
An unannounced monitoring inspection took place this morning. There were 25 children present with 4 staff. I observed a teacher reading "The Grinch Who Stole Christmas" to a circle of children. I also saw them working with Legos and puzzles and coloring. Record keeping was well-organized. No children's medications were on-site. Based on observation during today's inspection, there were some areas/equipment that were not maintained in safe condition. Evidence: There were some splintery areas observed on the two wooden play structures in the playground today; and a fireplace grate on the second floor was not securely fastened.

These will be repaired.

Description:
Based on a review of a sample of children's files and staff confirmation today, there was no documentation that the center has requested annual updates to the child's record as required. Evidence: The records from Child # 1, 2, and 3 did not have documentation of updates to the files as required.
Plan of Correction:
Parents have given us verbal updates previously but we will have them check emergency contact information and sign on an annual basis..
