Comments:
An unannounced monitoring inspection was conducted from 12:05 pm to 1:10 pm. At the time of entrance, 7 children totaling 21 points were in care with the provider and assistant provider. The record sample size consisted of 5 children's records and the provider's record and the assistant's record. The children in care were observed engaging with the provider, lunch, free play, and nap. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.
Based on record review and interview it was determined the provider did not ensure that each child's record contained all of the required information. Evidence: 1. The record for child #1 did not contain proof of identity. 2. The record for child #2 did not contain the medical insurance policy number and the address for one of the two required emergency contacts. 3. The record for child #5 did not contain: - Child's full name, nickname (if any), sex, address, and birth date; - Emergency contact information including - Name, address and telephone number of each custodial parent's place of employment; - Name, office address and telephone number of the child's physician; - Name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; - Information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation; - Name and policy number of the child's medical insurance, if applicable; - Names of persons other than the custodial parents who are authorized to pick up the child; - Appropriate legal paperwork when a custodial parent does not authorize the provider to release the child to the other parent; and - Chronic physical problems, pertinent developmental information, and any special accommodations needed 4. The provider confirmed that this information was not available during this inspection.

Description:
Based on record review and interview it was determined the provider did not ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. Evidence: 1. The record for child #5(enrolled 2/27/23) did not contain the required immunization documentation prior to attending. 2. Provider confirmed this documentation was not available during this inspection.
