Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A code compliance inspection was initiated on 02/11/2021 and concluded on 02/11/2021. The director was contacted by telephone to initiate the inspection. There were 70 children present and 12 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review, the center did not ensure two of three staff records had documentation of a completed sworn statement or affirmation prior to the first day of employment or central registry results within 30 days of employment. Evidence: 1. The record of staff #3 (start date: 8/19/2019) did not have documentation of central registry results. 2. The record of staff #1 (start date: 8/19/2018) did not have documentation of a completed sworn statement or affirmation.
Plan of Correction:
Staff #3 has filled out document and it was mailed on February 16, 2021.
Office staff will make sure documents are done before employment begins and I will check behind them. February 15, 2021

Description:
Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children. Evidence: During the inspection of three staff records and interview with the center representative, it was discovered there were no staff that received training in daily health screenings. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings.

1 staff member from the office is scheduled for that class.

Description:
Based on record review, the center did not ensure two of three staff records contained documentation of fingerprint based national criminal history search results prior to employment. Evidence: 1. The record of staff #2 (start date: 1/18/20) did not have documentation of fingerprint based national criminal history search results. 2. The record of staff #3 (start date: 8/19/19) had documentation of fingerprint based national criminal history search results dated 1/29/2020.
Plan of Correction:
Staff #2 is scheduled to be fingerprinted. February 18, 2021 @ 1:30
My office staff has been educated on staff forms that are needed to begin working. February 16, 2021
