Technical Assistance:
Discussion with the Provider of the Business Entity page that was submitted with the Renewal Application. A copy of the Articles for Organization are still needed. Documentation that this was filed with the SCC was given. However, it is the actual Articles of Organization that is needed versus a confirmation that it was filed.
A blank copy of a request of the central registry (CPS check) was sent to the Provider. Several checks are about to expire come early next year.
Comments:
A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.
This inspection was initiated and concluded on 10/26/2021. The Provider was contacted and a virtual inspection was conducted. There were 11 children present (34 points), ranging from infant to preschool-age, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (no medication on site); special care and emergencies; and nutrition. A total of 5 children records, 2 household records, and 4 staff records were reviewed.
Information gathered during the inspection determined that there was one (1) non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the program.

Based on review, 1 of 5 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who is not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.
Evidence - 1. On the date of inspection (10/26/2021), the current immunization record on file for Child #1 was dated - 04/06/2021. 2. It was confirmed that on the date of inspection (based on documentation on file) that Child #1 was under 2 years of age.
Plan of Correction:
The Parent of Child #1 shall be contacted and an updated immunization page shall be obtained. This updated documentation shall be placed in the child's file.
