An unannounced monitoring inspection was conducted on 11/23/2020 and 12/8/2020 to investigate a self-reported incident regarding staffing and supervision. Interviews were conducted and documentation was reviewed. It was determined that there was insufficient corroborating evidence to substantiate the allegation. There were no violations issued as a result of the inspection. An exit interview was conducted on 12/15/2020.
