Comments:
An unannounced complaint inspection was initiated on 6/3/2024 with the local department of social services to investigate an allegation received by the department related to supervision, forbidden actions, and procedures for emergencies. Additional interviews were conducted on: 6/12/2024. Based on evidence gathered during the inspection from interviews, observations, and a review of records, reports and digital documentation, the portion of the complaint regarding supervision was determined to be valid. The portion of the complaint regarding forbidden actions and procedures for emergencies was determined to be not valid. A violation notice was issued to the facility. Description: Based on interviews and a review of documentation, the center failed to inform the department?s representative of an injury to a child that occurred while the child was under the supervision of the center and required outside medical attention. Evidence: On 1/9/2024, child #2 sustained a "nursemaid" elbow injury that required medical attention. Staff confirmed that they did not complete the online IROMT form or notify the Department.

Description: Based on interviews, observations, and a review of records, reports and digital documentation, it was determined that staff supervising children did not always ensure their care, protection and guidance. Evidence:
1) On 5/3/2024, between approximately 7:52am-12:30pm, child #1 (age 5) sustained a black eye. Staff could not explain how the injury occurred, but confirmed the child arrived without the injury and were unaware of it until the parent pointed it out.
2) Child #1 stated that the injury occurred when another child pushed them on the slide, and they hit their head on the slide/tunnel. Multiple teachers were outside on the playground at the time, but stated they were also involved in setting up for the graduation ceremony.
3) On 5/24/2024, between 7:30am-3:09pm, child #1 sustained a "goose egg" on their forehead. Staff could not explain how the injury occurred, but confirmed the child arrived without the injury and were unaware of it until the parent pointed it out.
4) Child #1 stated that another child hit them in the head with an object when in the preschool classroom. Child #1 was in the preschool room with approximately 30 children and three teachers from approximately 7:30-10:36am, before moving to the school age room.
