Comments:
An unannounced monitoring inspection was completed on 03/02/2020 from 9:15 A.M.-12:30 P.M. At the time of the inspection 35 children were in care with six staff and one administrators present. Five children records, five staff records, evacuations drills, first aid kits, emergency supplies, fire inspection, health inspection, classroom observations in all classrooms, outdoor play area, and posted required information were reviewed. Children were observed during small groups participating in a science activity, a large group activity learning about plants/seeds, storytime, free play, music and movement activities, listening activities, handwashing, snack time, outdoor play time, and departure. Exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-185-60-A
Description: Based on review of children files, the center failed to ensure all required information was in a child's file.

Evidence:
1. Child #5's file only contained one of the two required emergency contacts.
2. Staff verified that Child #5's record only contained one emergency contact.

Plan of Correction: Administration will contact the parent at pick-up and request a second emergency contact for the child's file.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location.

Evidence:
1. In the restroom in Classroom #1 there was a container of Clorox Disinfectant Wipes sitting on top of the shelf that houses diapers.
2. The warning label states "keep out of reach of children", and hazardous to human and animals.
3. Staff verified that the Clorox Disinfectant Wipes were not in a locked location.

Plan of Correction: Staff immediately placed the Clorox Disinfectant Wipes into a locked location.
