There were 14 children present with four classroom staff , a cook and an administrator. The children were playing outdoors and eating lunch. Three child records and four staff records were reviewed. Evidence: 1. There are four of the required six inches of resilient surface material under the climber. Plan of Correction: The site supervisor reported that more resilient surface material has been ordered.
