Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was conducted on 6/11/2020. The director was contacted by telephone to initiate the inspection. There were 37 children present and 8 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Friday 6/19/2020. Based on record review, 1 of 3 children's records did not contain 2 complete emergency contacts to include name, address, and telephone number.
Evidence: Child #1's file was missing a telephone number for both emergency contacts and an address for one emergency contact.
Plan of Correction:
The phone # was added to Child #1's folder on 6/15/20. Contact info is also complete.
