Comments:
An unannounced monitoring inspection was conducted from 9:00 am-11:00. There were a total of 33 children with 5 staff members present. Children were observed during snack time, hand washing,engaged in group activities including story time and rhyming game. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Based on a review of children records, the center has not maintained a record for each child with the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. Evidence: Child #1, date of enrollment 8/2019 does not have documentation of 2 people to contact in an emergency, only one name and number was listed.
