Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding updated regulations in child/staff ratios, CPR certification, out of state searches, and accessing the department's website for notices, forms, and additional information. A copy of the new regulations was given to the director during this inspection.
Comments:
An unannounced monitoring inspection was conducted today, August 10, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at 1:00pm and concluded at approximately 3:00pm. Today approximately 60 children (ages 2.5-5 years) were in attendance being supervised by ten staff with the correct child/staff ratio in each classroom. Administrative records, children's classrooms, restrooms, sanctuary, gymnasium, cafeteria, outdoor playground, and common areas were inspected. See attached notice for the violation found during this inspection.

Based on the inspection of the children's playground, the center did not ensure the playground resilient surfacing was maintained.
Evidence:
During the inspection of the large playground, it was discovered the pea gravel has compacted and become a hard surface under the slides and swing set causing a hazard to children using the playground equipment. As a result, additional resilient surfacing (pea gravel) is needed under the two green slides and swing set.
Plan of Correction:
Per director: We are aware and in the process of ordering additional pea gravel for the children's playground.
