Comments:
The licensing inspector conducted an unannounced complaint inspection on October 24, 2019 in response to a complaint received by our department on October 9, 2019. The inspector was on site from approximately 11:45 AM to 1:30 PM. The allegations of the complaint were inadequate supervision, a failure to maintain a record of children's injuries, maintenance, and good character. Classroom observations were made, interviews were conducted, and records were reviewed. The preponderance of the evidence gathered during the investigation supports one of the allegations and the complaint is determined valid. See the violation notice for more detail.

Complaint related: Yes
Description: Based on a review of documentation, a review of photographs, observations, and interviews; the center did not ensure that children under the age of ten shall be within actual sight and sound supervision of staff. Evidence: 1. The licensing inspector observed a photograph of the Toddler room showing five children present with no staff present. 2. The licensing inspector observed that the attendance records for 9/27/19 at 4:43 PM indicated there were 5 children present in the Toddler room, and 2 children present in the Preschool room. 3. Staff interviews with the licensing inspector indicated the following: Staff described the late afternoon of 9/27/19 as "chaotic." Staff #2 left the Toddler room for 5 to 10 minutes in order to assist a family at the front entrance area of the center. When Staff #2 left the Toddler room, Staff #3 was supervising the Toddler room and the Preschool room at the same time by standing in an adjoining doorway between the classrooms. Staff #3 left the doorway to assist a child in the bathroom of the Preschool room. While Staff #3 was in the Preschool bathroom, a parent arrived in the Toddler room for pick up and was unsupervised with the children in the Toddler room. 4. The licensing inspector observed the bathroom located in the Preschool room is located in an area that would prevent the sight and sound supervision of the children in the Toddler classroom.

Plan of Correction: Per administration, "All children will be in sight and sound under the age of 10. Have a scheduled time to meet with parents. Teachers will
notify center director or designee when assistance is needed or a break is needed."
