Comments:
The licensing inspector conducted an unannounced monitoring inspection on 10/04/2019 from 11:18am to 2:12pm. The children were observed playing outside, reading, singing, dancing, finishing lunch and napping. A formal observation was completed with Room #2 and Room #5. Lunch consisted of, chicken patties, pineapples, greens and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and the playground was inspected today.
Five children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center?s injury prevention plan was updated 12/2018.
The center's first aid kit and emergency supplies were inspected. Last emergency drill: 09/18/2019 Last shelter-in-place drill: 06/03/2019 Last fire inspection: 01/15/2019 Last health inspection: 06/04/2019
Today, the following child to staff ratios were observed: Rm. #2 (Fours) ? 10:1 Rm. #3 (Threes, Fours and Fives) - 15:2 Rm. #4 (Threes and Fours) - 17:2 Rm. #5 (Fours and Fives) - 10:1
The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: The inspector observed several pieces of sharp broken glass on the center's playground near the basketball goal.
Plan of Correction:
Will check playground in the morning and have staff recheck prior to play.
