Thank you for your assistance during this unannounced complaint inspection. This on-site visit was conducted on August 24, 2022 from 1 p.m. through 2:50 p.m. This inspection was initiated as a result of a complaint received by the Valley Licensing Office on June 21, 2022. Observation and staff interviews were conducted relating to the allegation of physical plant and diapering. The information gathered during this investigation does not support the allegation.
