Comments:
An unannounced monitoring inspection was conducted with the director on June 4, 2024, beginning at 4:05 p.m. and ending at 5:25 pm. There were 26 children, ages between 5 months and 5 years old, in the direct care of 4 staff. The children were observed playing outside. Five children's records were reviewed and in compliance. Three staff records were reviewed and in compliance. Five medications and two allergy care plans were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The most recent practice emergency evacuation drill was documented as 5/27/2024. The most recent practice shelter-in-place drill was documented as 3/6/2024. The most recent practice lockdown drill was documented as 3/1/2024. The most recent health inspection was dated 2/29/2024. The most recent fire inspection was dated 3/14/2024.

Based on observation, not all areas and equipment inside and outside of the center were maintained in a clean, safe, and operable way. Evidence: The changing pad in the infant classroom had small particles and debris on it. The wooden gate on the toddler playground had three rusty nails protruding out of the fence. A full panel of the wooden gate was leaning towards the wooded area behind the center. There was a gap between the bent section of the wooden gate and chain link fence creating an entanglement hazards.
