Comments:
An unannounced complaint inspection was conducted on 12/13/2019 from 12:15 P.M.-1:15 P.M. The inspection was in response to a complaint that was received in the Valley Licensing Office on December 10, 2019. The complaint was in regard to behavioral guidance. There were three staff members interviewed, two staff records, one child record, one injury report, and attendance log were reviewed. The information gathered during the investigation does support the allegation, so the complaint is determined to be " valid." Exceptions to compliance are noted on the violation notice.

Based on staff interviews, the center failed to use behavioral guidance that was constructive in nature age and stage appropriate, and intended to redirect children to appropriate behavior and resolve conflicts.
Evidence: 1. On 12/9/2019 Staff #2 and Staff #3 were in Classroom #1 with five children, ages 12 months-24 months, all children and staff were sitting on the floor playing with blocks. Child #1, (age 17 months), threw a block that landed near Staff #2. Staff #2 tossed the block back to Child #1. The block hit Child #1 on the cheekbone right underneath the eye and left a bruise. 2. Staff #2 stated that during the interaction she stated to Child #1 "we do not throw blocks". 3. Staff #1, #2, and #3 all verified that Staff #2 had tossed a block back to Child #1 and hit him on the cheekbone leaving a bruise underneath the eye.
Plan of Correction:
Staff #2 has been trained on how to handle a child throwing blocks. We discussed leading by example and using words to build with blocks. Meeting was 12/17/2019. Discussion will be had for staff during a staff meeting on 1/14/2020.
