Comments:
An unannounced subsidy health and safety inspection was conducted on 11/14/2019 from 4:00 pm - 5:05 pm. A monitoring inspection was concurrently conducted as this program is a licensed child day center. At the time of entrance there were 45 school age children in care with three staff. Children were observed during afternoon program time which included snack and organized games in the gym. Records were reviewed for nine children in care. Medication and emergency supplies additionally reviewed.
Based on record review and interview, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: 1. There was no written allergy care plan on file for child 1. a. The enrollment record and physical for child 1 indicated a diagnosed food allergy to peanuts and tree nuts. 2. The program director confirmed that there was not a written allergy care plan on file for child 1.
Plan of Correction:
Have parent provide allergy action plan from physician.
