Comments:
An unannounced complaint inspection was conducted on 09/23/2022 in response to a complaint regarding attendance documentation and care and protection of children. Based on the information gathered which included staff interviews, observation of the program and a review of written records the complaint is found to be valid in the area of attendance documentation. The allegation regarding care and protection of children can not be substantiated and is found to be not valid.
A violation of Standard 80.A is listed on the violation notice issued to the facility.
Based on record review and interview, the center failed to ensure for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: 1. Written attendance records show that attendance was not documented, at the time of arrival & departure for all children in care on 09/06/2022: a. On 09/06/2022 4 children attending the before school program were not checked out at departure into the public school. b. On 09/06/2022 3 children were not checked into the program until 3:48 pm however, they arrived at approximately 2:45 pm. c. On 09/06/2022 3 children attending the after school program were not checked out at departure to home. 2. Written attendance records show that the departure of 5 children was not documented on 09/08/2022. 3. Written attendance records show that the departure of 4 children was not documented on 09/09/2022. 4. Administrative staff confirmed the accuracy of these attendance records.
Plan of Correction:
Administrative staff stated that all staff will be retrained on the importance of maintaining accurate attendance. Staff will be reminded to make sure attendance is noted at arrival and departure for both the morning and afternoon programs.
