Comments:
An unannounced subsidy health and safety inspection was conducted on 11/13/2019 from 3:15 pm - 5:05 pm. At the time of entrance there were 47 school age children in care with four staff. At 3:38 pm there were 51 children in care with five staff when additional children arrived from chorus. Children were observed engaged in afternoon program time which included art, homework and organized games in the gym. Records were reviewed for nine children in care.
Based on record review and interview, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: 1. There was no written allergy care plan on file for child 6 who was in care during the inspection. a. The parent of child 6 indicated on the enrollment record that the child had a diagnosed food allergy to tree nuts, shell fish and berries. b. The parent of child 6 indicated on the enrollment form that the child required emergency medications in the event of contact with the above offending foods. 2. Administrative staff confirmed that a written allergy care plan had not been obtained for child 6.
Plan of Correction:
