A subsidy inspection was initiated on May 9, 2022 and concluded on May 23, 2022. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Based on record review the vendor failed to ensure that all staff that work directly with children have cardiopulmonary resuscitation and first aid. Evidence: 1 - The record for staff 3 did not contain documentation that first aid or CPR training had been completed. a. Staff 3 had a documented date of hire as January 12, 2022.
