An unannounced record review inspection was conducted at the central location from 9:30am through 11:07pm. Four staff records were reviewed. Violations regarding documentation of staff qualifications and tuberculosis screening were identified and appear on the violation notice.
Based on record review the center failed to ensure each staff member obtains documentation of a negative tuberculosis screening within the allotted time frames. Evidence: 1 - The record for staff 2 does not contain any documentation that a tuberculosis screening was completed. a. Staff 2 has documented date of hire as January 29, 2019. 2 - The record for staff 3 does not contain any results of tuberculosis screening. a. Staff 3 has documented date of hire as September 3, 2019.

Staff 1 was observed working in the position of program director during an onsite inspection on September 24th and 25th, 2019. b. Staff 1 stated that he was the program director for the site during the inspection on September 24th and 25th, 2019. c. Staff 1 has documented date of hire as August 22, 2018. 2 - The record for staff 2 states that staff 2 has a job title of program leader. However, there is not any documentation that staff 2 has obtained the required training to support this job title. a. Staff 2 has documented date of hire as January 29, 2019. 3 - The record for staff 3 state the staff's job title is program leader. However, there is not any documentation that staff 3 obtained the required training to support this job title. a. Staff 3 has a documented date of hire as September 3, 2019.
