The Licensing Inspector initiated and completed an unannounced monitoring inspection on 6/23/2023. The inspector was on site from 12:20pm until 12:45pm.
Inspector was on site to review providers records for renewal application.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the provider failed to obtain an updated search of the criminal history check after 5 years.
Evidence: During the inspection on 6/23/2023, the provider failed to provide an updated Criminal History check for provider #1. The last Criminal History check for provider #1 was 5/24/2018.
Plan of Correction:
Fingerprints were received on 6/29/2023.
