Comments:
A Subsidy Health and Safety inspection was conducted on September 11, 2019 to review supplemental health and safety requirements. Five staff were present with twenty-three children. Five children and five staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on observation and review, facility did not ensure there is a current written list of all children's allergies, sensitivities, and dietary restrictions. The list shall be dated and kept confidential in each room or area where children are present.

Evidence: The following was found:
1. The allergy list was posted on a door in the classroom visible to the public.
2. The allergy list was not dated.
3. The records for child #1 and child #2 had documentation on medical forms of foods they were allergic to and the foods were not all noted on the allergy list.

Plan of Correction: The Recreation Center Supervisor will ensure the allergy lists is updated, dated and unavailable to the public but accessible to all staff.
