Comments:
An unannounced monitoring inspection was conducted today from 11:00am-2:30pm. There were children 82 (infants-5yrs.) directly supervised by 15 staff. The physical plant, 9 staff records, 9 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, participating in learning activities, being bottle fed, napping, and eating lunch. Hand-washing procedures were also observed. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Based on record review, children's records did not contain all the required information.
Evidence: Child#8's record does not contain the father's place of employment and his work phone number. Child#9's record does not contain the father's place of employment.
Plan of Correction:
During orientation we will make sure that we have both parents place of employment.
