Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 6/22/2020 and concluded on 6/24/2020. The Director was contacted by telephone to initiate the inspection. There were 33 children and 4 staff. The inspector emailed the Director a list of items required to complete the inspection. The inspector reviewed 3 children's records and 3 staff records submitted by the facility to ensure documentation was complete.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and interview, the center has not obtained immunizations for each child before the first date of attendance. Evidence: Child #3 (date of enrollment: 2/11/2020) file does not contain documentation of any immunizations. The center Director confirmed the immunizations were not yet obtained.
Plan of Correction:
I will get in touch with the parents to get the status on the health forms and require the missing documents to be provided as soon as possible.

Description:
Based on record review and interview, the center has not obtained a physical exam for each child within the first 30 days of attendance. Evidence: Child #3 (date of enrollment: 2/11/2020) file does not contain documentation of a physical exam. The center Director confirmed the physical has not yet obtained.

I will get in touch with the parents to get the status on the health forms and require the missing documents to be provided as soon as possible.

Description:
Based on record review and interview, staff have not resubmitted a negative tuberculosis (TB) screening every two years. Evidence: Staff #3's most recent TB screening results are dated 2/15/2018.
Plan of Correction:
This staff person is not here this week. I will require them to bring me an updated screening before they can come back.
