63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted Monday, August 12, 2019 to determine the center's compliance with religious exempt requirements. The inspection was initiated at approximately 10:45 AM and concluded at approximately 1:00 PM. A total of 20 children were present and supervised by five staff members. The required staff-to-child ratios were being maintained.

Children were observed having water play outside, having a book read to them, working puzzles, watching children?s programs on T.V., washing hands and taking bathroom breaks, eating lunch and resting at nap time. All areas of the center were inspected including the classrooms, hallways, bathroom and outdoor play area. Five children?s records and five staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of five staff records, the center did not ensure that all staff and volunteers had a search of the central registry within the first 30 days of employment or volunteering. Evidence: 1. The records for Staff #4 (hire date 12/15) did not contain central registry results. The central registry findings could not be located during the inspection. 2. The local agency was called to see if the central registry request had been received. An official from the local agency said the records did not go back to 2015.

Plan of Correction: I will check with previous director to see if the central registry finding is filed somewhere separate. We will also request a new central registry check since it will be close to time to update it.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of five staff records, one record did not contain the required updated background checks. Evidence: The record for Staff #3 (hire date: 02/28/01) contained a central registry finding dated 05/07/01 and a sworn statement dated 02/28/2001. Effective July 1, 2017, staff at all religiously exempt child day centers must repeat background checks every five years.

Plan of Correction: I will have the sworn statement updated today and send of a request for an updated central registry finding tomorrow.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of five records and interview, the center did not obtain satisfactory results of the finger-print based national criminal background check for one staff prior to employment. Evidence: The record for Staff #2 (hire date 06/11/2019) contained satisfactory results of the finger-print based national criminal background check dated 06/20/2019.

Plan of Correction: We will not allow new staff to start before receiving the results of the national background check.
