The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on November 13, 2019 in reference to allegations of building maintenance and behavioral guidance. The inspection was completed on December 3, 2019. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports one of the allegations; therefore, the complaint is determined to be valid. Based on staff interviews, the center did not ensure that the areas of the center shall be maintained in a clean, safe and operable condition. Evidence: During interviews with the licensing inspector staff reported an ongoing odor for approximately two weeks in the school age bathroom. Staff interviewed reported various cleaning treatments have been attempted to remove the odor. Staff interviewed reported further maintenance measures in addition to surface cleaning are being investigated by administration.
Plan of Correction:
Administration has an appointment to receive an estimate for any needed repairs to be made in the school age bathroom on 12/10/19. Administration will continue to clean the restroom daily and have also begun using an enzyme spray daily in order to improve odor concerns until the necessary repairs are discovered and completed. Administration estimates the repairs should be completed in an approximate two week timeframe.
