A subsidy health and safety (SHSI) inspection was initiated on May 23, 2023 and concluded on May 23, 2023. There were 92 children present with 16 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on staff record review, the provider failed to ensure that staff working directly with children shall complete annually the department's health and safety update.
Evidence: The record for staff #2, did not contain documentation of the annual health and safety update since 04/28/22. .The record for staff #5, did not contain documentation of the annual health and safety update since 5/22/21.
Plan of Correction:
Staff #2 completed this training om 06/16/2023, and staff #5 did so on 05/10/2023
New management has now created a chart with staff training fates to ensure that staff complete any necessary professional development on an annual basis. Staff will receive 30 days notice from management when training must be completed for renewal or based upon tenure at the facility (16 -24 hour based on position).
