Comments:
A monitoring inspection was initiated and concluded on June 4, 2024. There were 12 children in care with two staff. The children ranged in age from three to five years old. Children and staff records were reviewed. Children were observed in teacher and self-directed activities. Staff records are housed at a central location and will be reviewed at a later date. Children's records were reviewed. Additionally, required postings, emergency evacuation drills and emergency equipment, and medication were reviewed. A walking tour of the center spaces inside and outside was completed to include the outside playground. A single violation regarding trip hazards on the playground was observed, discussed with onsite staff and appears on the violation notice.

During the inspection three additional proposed classroom spaces were viewed and measured for a requested modification to increase the center capacity. Each room has a toilet and sink within the room. However, only classroom measurements were taken because the classrooms were not set up with equipment and materials. A virtual visit may be in order in the future once the classrooms are appropriately outfitted to care for children.

Evidence:
On the playground there were approximately eight large bolts protruding up above the boarder surrounding the playground posing a trip hazard.
