Comments:
A renewal inspection was initiated on 12/9/20 and concluded on 12/15/20. The director was contacted by telephone to initiate the inspection. There were 17 children with 2 staff present.
The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children,3 staff and 3 board officer's files.
The information gathered during the inspection determined there were two violations with applicable standards or law.
Based on interview, it was determined that there has not been an annual fire inspection report. Evidence: The last documented fire inspection report is dated 5/17/18.
Plan of Correction:
We will find a more up to date report or contact our local fire official to complete an inspection.

Description:
Based on a review of three staff and 3 board member's files, it was determined that one staff had not obtained a repeat background check every five years. Evidence: Staff A's Central Registry Finding Report is dated 7/1/15.
Plan of Correction:
We will obtain a new background check.
