Comments:
An unannounced monitoring inspection was conducted on 06/27/2023. There were 8 children, ages 2 years to 8 years, totaling 12 points and one caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records, one caregiver, and one household member record were reviewed. The children were observed during outdoor time, and teacher directed time. The inspector discussed the following with the provider: children?s records, USDA, supervision and emergency plan. The inspector arrived for the inspection at 10:30 am and departed at 12:15 pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review and interviews with caregivers, the provider failed to obtain for each caregiver and adult household member a current report of tuberculosis screening, every two years from the date of the first screening.
Evidence: The record for Household Member 1 contained a report of tuberculosis screening dated 04/29/2021. Caregiver 1 confirmed it was the most recent report of tuberculosis screening.

Provider will obtain TB Screening for Household member.

Description:
Based on observation and interviews with caregivers, the provider failed to ensure that caregivers shall obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.
Evidence: The record for Caregiver 1 did not contain any training hours obtained in the previous 12 months. Caregiver 1 confirmed 16 clock hours of annually training documentation was not available for review.
