Comments:
An unannounced monitoring inspection was conducted on 12/27/2023. There were 3 children, ages 3 years, totaling 6 points, and one caregiver present during the inspection. The children were observed during child-directed and teacher-directed activities. The inspector discussed the following with the provider: FieldPrint and CRS Portal. The inspector arrived at approximately 9:00 am and departed at approximately 11:45 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on record review, the provider failed to ensure that required background checks are repeated every 5 years per the Code of Virginia.
Evidence: 1. The most recent search of the central registry for Caregiver 1 was dated 09/21/2018; there was not a 2023 repeat search of the central registry available for review during the inspection. 2. The most recent eligibility letter for Caregiver 1 was dated 09/24/2018; there was no 2023 employment eligibility letter available for review during the inspection. 3. The most recent search of the central registry for Household Member 1 was dated 09/27/2018; there was not a 2023 repeat search of the central registry available for review during the inspection. 4. The most recent eligibility letter for Household Member 1 was dated 09/24/2018; there was no 2023 employment eligibility letter available for review during the inspection.

Description:
REPEAT VIOLATION Based on document review and interview with caregivers, the provider failed to ensure that wood burning stoves and fireplaces and associated chimneys shall be inspected annually.
Evidence: Caregiver 1 confirmed there was no annual chimney inspection for the wood burning stove available for review. The provider confirms the word burning stove gets used most mornings. The inspector observed the chimney in use.

Provider will contact someone to conduct inspection.
