A complaint investigation was initiated on August 22, 2022, with another local agency and ended on September 9, 2022, regarding a 1 year old child was grabbed and pulled by a staff member. Interviews were conducted and documentation were reviewed. There was not a preponderance of evidence to support the allegation regarding care and protection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff records review, the center did obtain required documentation for staff records. Evidence: 1. Staff #2 (date of employment 7/24/2019) did not have two or more references as to character and reputation as well as competency was checked before employment.

Description:
Based on staff interviews, the center's staff did not follow a flexible daily scheduled for infants based on their individual needs. During the day, infants shall be protected from older children. Evidence: 1. Staff #1 clocks in for work around 7:30 a.m.. Staff will care for their two children (infant and two year old) along with 6 infants for about 5 to 10 minutes when Staff #2 arrives. 2. Staff #1 would have their two children; carrying one child and the other child walking beside staff. There would be 6 infants who in a buggy during this time. 3. The buggy is used during arrival 7:00 am to 8:00 am (per infant daily schedule) 4. The staff and ratio for the infant classroom is 1 staff to 5 children. Staff was out of compliance with ratio by caring for their two children infant and two year old) and infants.
