Comments:
A monitoring inspection was conducted today. There were 5 children and 2 staff present during the inspection. Observation of the infant classroom was conducted. A review of 2 children records and 2 staff records were reviewed. Documents received on review of head injury policy and parent communication,

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-60-A
Description: Repeated violation
Based on children's records review, 1 out of 2 records, the center did not maintain for each enrolled child record the required information the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Evidence:
1. Child #1 (date of enrollment 10/20/2023) did not have two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Had parent fill emergency contact 2 out on form.

Both managers will triple check emergency contact forms prior to starting.

Standard #: 8VAC20-780-520-A
Description: Based on observation, the center did not ensure all nonprescription drugs and over-the-counter skin products shall be used in accordance with the manufacturer's recommendations. Nonprescription drugs and over-the-counter skin products shall not be kept or used beyond the expiration date of the product.
Evidence:
In the infant classroom, one child had expired diaper ointment (6/2022).

Plan of Correction: Returned ointment to parents. Teachers will be more diligent checking dates if ointments is not brand new. created a ointment log to keep track of expiration dates.
