The licensing inspector conducted an unannounced monitoring inspection on Friday, July 19, 2019 from approximately 9:50am to 11:25am to determine the provider?s compliance with licensing standards at the family day home. During the inspection, there were 6 children present totaling 11 points being cared for by two caregivers. Upon the inspector?s arrival, the licensing inspector observed the caregivers and the children in the main play area of the family day home. The licensing inspector observed the children engaged in play with their peers, coloring, and interacting with the caregivers. The menu was posted and today?s lunch consisted of fish sticks, wheat bread, carrots, lettuce, tomato, and milk. The licensing inspector interacted with the children when appropriate throughout the inspection. The caregivers were observed having positive interactions with the children in care. The required postings to include the license, emergency contacts, and recent inspection summary were inspected and found to be in compliance. The first aid kit and emergency supplies, outdoor playground area, pool, designated childcare areas, diapering station, the bathroom, and the kitchen were inspected. During the inspection, five children?s records, two caregivers? records, one household member?s records, and three pets? records were reviewed for compliance of the licensing standards. The last documented evacuation drill was on July 15, 2019 and the last documented shelter-in-place drill was on July 15, 2019. Based on a review of documentation and interview on 07/19/2019, the provider did not maintain a current Report of Tuberculosis (TB) Screening form for one out of two caregivers every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence: 1) The record for Caregiver #1 contained a TB screening test dated 02/08/2017. 2) During interview, Caregiver #1 confirmed the TB screening for Caregiver #1 expired on 02/08/2019.
Plan of Correction:
Caregiver #1 has scheduled an appointment for July 24, 2019 at 8:00 am to complete the required TB screening/test.
