Comments:
A renewal inspection was initiated and concluded on 4/13/2022. There were 3 children present, with the provider and 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, personnel, household members, physical health of caregivers and household members, physical equipment and environment, care of children, emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-800-B
Description: Based on record review, the provider did not review the emergency plan at least annually.

Evidence:
1. The last documented review of the emergency plan was dated 03/11/2021.

Plan of Correction: I will review it tonight.

Standard #: 8VAC20-800-180-A
Description: Based on record review, the provider did not obtain a current Report of Tuberculosis Screening form every two years for 2 caregivers.

Evidence:
The most recent tuberculosis screening in the record for Caregiver #1 was dated 03/13/2019.
The most recent tuberculosis screening in the record for Caregiver #2 was dated 01/24/2019

Plan of Correction: We will complete them at our yearly physicals that are coming up soon.

Standard #: 8VAC20-800-240-A
Description: Based on observation, equipment inside of the home was not maintained in a safe and operable condition.

Evidence:
A dime sized hole containing sharp edges was observed on the side of a toy kitchen. The hole could cause a potential safety hazard to children.

Plan of Correction: I will fix the hole on Saturday.
